Details: This office represents an employee injured in an explosion involving “Boiler 9” at the University of Connecticut Central Utility Plant on the Storrs campus on or about May 15, 2026,
at approximately 11:35 p.m. (the “Incident”). Pursuant to the Connecticut Freedom of Information Act, Conn. Gen. Stat. § 1-200 et seq., I request copies of the following records
concerning the Incident:
1. All incident reports, case reports, and supplemental reports, including any report number(s) assigned to the Incident.
2. All computer-aided dispatch (CAD) records, call logs, radio logs, and 911 recordings or transcripts relating to the Incident.
3. All fire department response records, including the NFIRS incident report, apparatus/run reports, and any EMS/patient-care report prepared in connection with the ambulance
transport from the scene. (Any required HIPAA authorization for patient-care records will be provided upon request.)
4. All fire marshal or fire investigation records concerning the origin and cause of the explosion, including reports, findings, notes, diagrams, and evidence logs.
5. All photographs and video taken at or of the scene, including body-worn camera footage of responding officers.
6. All witness statements obtained in connection with the Incident.
7. All records of referrals or notifications to any other agency concerning the Incident (e.g., the State Fire Marshal, CONN-OSHA, the Department of Administrative Services, or any other insurer)
