Details: This is a request to the University of Connecticut (UConn) under the Connecticut Freedom of Information Act (the FOIA), C.G.S. Section 1-200, et. seq. Connecticut Public and I hereby request that UConn make available for inspection and provide copies of the following the following categories of records with regard to Martin Kulldorff’s employment with the university:

Personnel Action Records
Employee History Cards
For reference, Mr. Kulldorff was an associate professor/basic science in what used to be named the department of Community Medicine and Health Care (now called Public Health Sciences). He was employed at UConn from Sept. 28, 1998 through Aug. 31, 2003.

To the extent that UConn claims that any record or a portion of any record is exempt from disclosure, please provide the basis for this position including the specific exemption(s) upon which it relies.
