Details: Dear Public Records Officer:

Under the Connecticut Freedom of Information Act (FOIA). C.G.S. § 1-200, et seq., I am writing to request the following public records:

Records of all Care Team Referral Forms and Community Standards Referral Forms submitted to the Office of Student Care and Concern between January 1, 2022 and January 1, 2023. I am requesting records of the initial reports filed (with appropriate redactions to remove personally identifying information), and the reported outcomes of each case (e.g. disciplinary or other administrative actions).

I request that all these documents be provided electronically in their original formats rather than in a physical format such as paper.

These records are in the public interest, because they deal with student and resident public safety. Should you find that these records are not in the public interest and decide to impose a fee, I request that you justify this decision, and itemize each charge in writing if the cost exceeds $20. However, I would also like to request a waiver of all fees in that the disclosure of the requested information is in the public interest and will contribute significantly to the public’s understanding of mental health on college campuses.

I am a researcher and a fellow with the Longmore Institute on Disability at San Francisco State University. This information is not being sought for commercial purposes.

If you deny any or all of this request, please cite each specific exemption you feel justifies the refusal to release the information and notify me of the appeal procedures available to me under the law. If you determine that a public record contains information which is subject to access as well as information which is not subject to access (e.g. protected information under FERPA), the law prohibits denying access to the record if the information which is not subject to access is able to be redacted.
