Details: Pursuant to the Connecticut Freedom of Information Act, C.G.S. §§ 1-200 et seq., and specifically § 1-210(a), I request copies of the public records described below. The University of Connecticut is a public agency within the meaning of § 1-200(1), and the Connecticut Veterinary Medical Diagnostic Laboratory is a unit of the University.
The records concern necropsies performed by the Laboratory on animals that were seized from me on July 10, 2024. I am the owner of those animals. The specimens were submitted under four accession numbers: 24-4050, 24-4231, 24-4242, and 24-4683. I have received reports corresponding to two of the four accessions. I have not received the other two.
I previously sent the Laboratory a preservation and litigation-hold letter regarding these same records, by electronic mail and by certified mail. This request is separate from that letter and is made under the Freedom of Information Act.
Records Requested
For each of accession numbers 24-4050, 24-4231, 24-4242, and 24-4683:
1. All reports of any kind, including preliminary, supplemental, amended, and final reports.
2. All necropsy, histopathology, laboratory, toxicology, and ancillary test results, including any results obtained from or reported by a referral or outside laboratory.
3. All submission forms, intake documents, and chain-of-custody records, including records identifying the submitting party and the date and time of submission.
4. All photographs and images.
5. All correspondence concerning the accession, including correspondence with any animal control officer, law enforcement agency, prosecuting authority, or veterinarian.
6. All billing records, including billing statement number PP24029958.
For CVMDL accessions 24-4050, 24-4231, 24-4242, and 24-4683, and for any other accession involving an animal submitted by or on behalf of NECCOG Animal Services, for the period June 1, 2024 to the present:
All communications, in any form, between CVMDL personnel and any person or entity outside CVMDL concerning these accessions, including but not limited to email, letters, facsimiles, text messages, voicemail, and notes or logs of telephone conversations.
All internal CVMDL communications concerning these accessions.
All communications with NECCOG Animal Services, Jennifer Hutchins, the Connecticut Department of Agriculture, Jeremy DePietro, any municipal or state animal control officer, any law enforcement agency, the Office of the State's Attorney, Halloran & Sage LLP, Attorney Carrie Burnsed, Norwichtown Veterinary Hospital, or Dr. Casey Pacheco.
All submission forms, transmittal letters, requisitions, intake documents, and chain-of-custody records, including any accompanying instructions or narrative.
